Key Responsibilities\n

  • Conduct comprehensive assessments to identify and address the specific mental health needs of patients.

  • Facilitate regular communication with supervisors, caregivers, and interdisciplinary team members to discuss patient progress and adapt treatment plans as necessary.

  • Develop and maintain a tentative schedule of appointments and follow-up activities to monitor work advancement.

  • Organize and coordinate home visits that include in-home observations, chart reviews, and interviews to ensure holistic care.

  • Ensure effective implementation of personalized treatment plans and monitor their execution throughout the care journey.

  • Participate in Interdisciplinary Team Meetings to collaboratively discuss patient cases and care strategies.

  • Document patient progress through monthly and quarterly notes, capturing important treatment developments and outcomes.

  • Train and certify team members on best practices for executing program plans and maintaining accurate documentation.

  • Provide ongoing support through follow-up interviews and assessments to ensure the continuous well-being of individuals.

  • Act as a liaison between clients and the mental health community, advocating for patients’ needs and development goals.

In this role, you will work with a diverse range of clients, including children, adolescents, adults, and families, addressing various mental health issues.
