Key Responsibilities\n

- Establish strong relationships within community resource groups, representing Redesign Counseling Hub during meetings and committees to advocate for the social needs of our patients.
- Provide crisis intervention and counseling to clients, actively assessing and treating a broad spectrum of emotional and behavioral issues.
- Communicate effectively with referring physicians regarding patient progress, challenges, and needs, ensuring an integrated approach to client care.
- Maintain timely and accurate clinical documentation including progress notes, treatment updates, and other required records in compliance with departmental policies.
